What the Test Actually Involves
Each item gets a visual inspection first, which is where the majority of genuine failures are found. Damaged leads, cracked plugs, exposed conductors, strained cord grips, missing pins and damage to the housing.
Then electrical testing appropriate to the class of equipment: earth continuity for earthed items, insulation resistance, and polarity where relevant. Portable RCDs additionally get a trip-time test.
Items that pass get a durable tag showing the test date, the retest date and who tested it. Items that fail come off the floor and get either tagged out or repaired, depending on what's wrong and whether repair is sensible.
The register is the part that matters for compliance. You get a full record of every item, its location, its result and its next test date, so when someone asks for evidence you produce a document rather than a shrug.
How Often It Needs Doing
Retest intervals depend on the environment and the type of equipment, and they're set out in the relevant standard rather than being a matter of opinion.
Broadly, the more hostile the environment, the shorter the interval. A construction site where leads get dragged across concrete and driven over is at the short end. An office where a monitor sits on a desk and is never moved is at the long end. Hire equipment, which gets used by people who don't own it, has its own requirements.
